What is Hospice?
Hospice is a comprehensive program that provides care to patients with a life-limiting, terminal illness and their families.

Most individuals would prefer, if possible, to spend their last days at home… alert and free of pain, among the people and things they love.

Hospice of Stokes County is dedicated to making this possible.

How does Hospice Provide Care?
A team of registered nurses, physicians, medical social workers, certified nursing assistants, chaplains and volunteers work together to keep the patients as comfortable and pain free as possible. Hospice staff focuses on the unique needs of patients and families, giving special attention to physical, emotional, spiritual and social needs.

What services does hospice offer?
The hospice team provides the following services based on patients/family needs and desires:
  • The patients personal physician provides medical care. The hospice medical director is available for consultation with the patient’s physician and for coordination with the team.
  • A registered nurse makes home visits as needed to assess the patient’s conditions. Hospice care is designed to alleviate pain and control the symptoms of the disease with expertise and compassion. The Hospice nurse consults with the patient’s doctors and other hospice professionals on an ongoing basis. The hospice nurse helps the patient and family members decide the course of their health care.
  • Registered nurses assist patients with all medical needs. They help educate those involved with the patient’s daily care and serve as a source of emotional support.
  • The nurse is in regular contact with the patient’s physician and will arrange special medical equipment as needed.
  • A 24-hour emergency on-call nursing service is provided to assist patients and family members.
  • Medical social workers help determine needs of patients and family members. They locate and coordinate community resources, helping the family solve problems associated with personal illness.
  • Nursing assistants make home visits to help with personal care.
  • Volunteers provide assistance at all levels of skill. All Hospice volunteers have completed a through training program.
  • Chaplains and other specialty volunteers are available when needed.
  • A bereavement program offers support to family members during the crucial year of adjustment following the loss of a loved one.
What does the hospice admission process involve?
Hospice will contact the patient’s physician to ensure he or she agrees that Hospice care is appropriate for the patient at this time. The Hospice care goals are set by the patient, family and the professional Hospice team based on individual patient and family needs. Hospice services are available to all eligible families without regard to race, age, sex, national origin, religious beliefs, or ability to pay.

How is hospice financed?
Hospice coverage is widely avaialble through Medicare and Medicaid, as well by most private insurance plans. Hospice may also supplement these forms of reimbursement by using contributions, gifts, and memorials.
